A Career Forged at the Intersection of Art and Science
Dr. Kotti’s path to surgery was shaped early by two seemingly opposing instincts: a rigorous scientific mind and a deep affinity for visual art. From adolescence, he was active in school science and physics clubs while simultaneously pursuing sculpture and the visual arts. He had initially hoped to attend art school. His parents advised otherwise.
That redirection proved consequential. In plastic surgery, Dr. Kotti found the discipline that united both halves of his intellectual identity — art and science in equal measure. He graduated summa cum laude with a Doctor of Medicine in 2006, then pursued advanced training across two countries:
- Diploma, French College of Plastic Reconstructive and Aesthetic Surgery (PRAS) — Paris, 2007
- Doctor of Philosophy (PhD) in PRAS — Medical School of Nice Côte d’Azur, 2008
- European Board Certification in Plastic, Reconstructive and Aesthetic Surgery (EBOPRAS) — Madeira, 2008
That final credential carried particular historic significance. Dr. Kotti became the first native Arab and African non-European to achieve EBOPRAS doctorate-level certification — a distinction that marked both personal achievement and a broader opening of the discipline’s highest tier to surgeons from outside its traditional European centre of gravity.
Leadership Across Three Decades of International Surgery
From 2009 to 2014, Dr. Kotti served as Head of the Plastic Surgery Unit at a national anti-cancer centre, where he led a programme spanning oncological reconstruction as well as aesthetic and restorative procedures.
In 2010, he became National Secretary of the International Society of Aesthetic Plastic Surgery (ISAPS), a position he held for eight years. Concurrently, he served on the executive committee of the International Confederation of Plastic Reconstructive and Aesthetic Surgery (ICPRAS) Humanitarian Section from 2011 to 2015.
His international practice expanded progressively. From 2014 onward, Dr. Kotti established a clinical presence in the UAE, working with Dubai Cosmetic Surgery and BR Medical Suites before joining Danat Al Emarat Hospital for Women & Children in Abu Dhabi as an expert consultant to build and lead a new plastic surgery unit. He subsequently joined the American Academy of Cosmetic Surgery Hospital in Dubai Healthcare City. As of 2025–2026, he practises primarily at Seline Clinic in Dubai.
He is a member of leading professional societies including ISAPS, ASPS (American Society of Plastic Surgeons), ASAPS (American Society for Aesthetic Plastic Surgery), and the Emirates Plastic Surgery Society. He has served as editor and peer reviewer for Aesthetic Plastic Surgery (the APS Blue Journal) and the Chinese Journal of Plastic and Aesthetic Research (PAR), and has been published in the Annals of Cosmetic Plastic Surgery and the European Journal of Plastic Surgery, among others.
Innovation in Breast Surgery — The Breast Key Concept
Dr. Kotti’s most internationally recognised area of specialisation is breast surgery — and specifically, his development of an entirely new analytical framework for understanding, categorising, and treating the breast.
In 2020, he authored “The Hidden Stories of the Breast” — a book that distils his clinical philosophy and introduces several concepts that have since influenced peers and practitioners across the discipline:
- Golden ratio analysis of breast morphology — applying the Fibonacci Phi number to define the proportions of the aesthetically ideal breast with mathematical precision
- The Breast Key (BK) concept — a new diagnostic and treatment algorithm that reframes how surgeons assess breast anatomy and surgical candidacy
- Redefinition of ptosis — a clinical and philosophical reframing of what constitutes a “normal” breast versus a pathologically sagging one, centred on the concept of normal natural ptosis — a framework designed to reduce patient anxiety around the entirely natural effects of age and gravity
