Buttock lipofilling: BBL
Buttock lipofilling or BRAZILIAN BUTT LIFT (BBL) is an aesthetic procedure that consists of injecting the patient’s autologous fat into the buttocks after sucking it from a donor area (hips, abdomen, thighs, etc.).
This procedure is completely natural and does not invite any foreign elements. It is done to reshape the buttocks and refine the general silhouette by Dr Kotti, providing a more contoured and balanced appearance — while simultaneously slimming the donor areas through liposuction.
Who is a candidate?
Buttock lipofilling is suited to individuals with:
- Flattened or undefined buttocks
- Difficulty building volume through exercise
- Sagging due to significant weight loss or aging

Buttock lipofilling is a surgical option to achieve the following aesthetic aims:

Steps of the buttock lipofilling operation
The plastic surgeon uses suction cannulas to remove fat from donor areas such as the back, thighs, abdomen… The fat is then purified and processed before being injected back into the buttocks.
The injection is carried out through fine incisions in several points in order to have a homogeneous and harmonious rendering. Finally, the incisions are sutured and dressed.
The postoperative course
After the surgery, a 24–48 hour hospital stay is required for monitoring. Recovery guidelines include:
- Avoid all pressure on the buttocks for 2–4 weeks
- Wear compression garments for 6–8 weeks
- Limit intense physical activity for the first week; resume gradually after 4–6 weeks
- Avoid sleeping on your back during the recovery period
- Maintain adequate hydration and protein intake to support healing
- Attend all scheduled follow-up appointments
Painkillers prescribed by the surgeon should be taken as directed. Prophylaxis against venous thrombosis is usually prescribed as well.
Buttock lipofilling: The result
The result of a buttocks lipofilling can be noticeable from the third week following the procedure. The final result will be appreciated from the 3rd month once the swelling has completely subsided and the fat has settled.
This procedure carries risks including infection, swelling, hematomas, fat embolism, scarring, potential shape asymmetry, and complications from anesthesia. Individual results vary per patient.
