Breast size reduction surgery removes selected tissue, fat and skin; the suitable approach depends on tissue composition, skin excess, symptoms and health.

The surgeon removes a planned combination of glandular tissue, fat and skin, then reshapes the remaining breast. The nipple and areola are normally moved on a tissue pedicle designed to preserve blood supply and, where possible, sensation and duct connections. Liposuction can refine selected fatty areas but cannot replace skin and gland removal in every patient.

A vertical pattern usually places a scar around the areola and down to the breast fold. An inverted-T or anchor pattern adds a horizontal scar within the fold and permits a larger adjustment. The choice depends on volume, skin excess, nipple position, tissue quality and circulation. Very large reductions may occasionally require a different nipple-management plan. Scars are permanent and mature gradually.

Breast reduction and lift usually form part of the same surgical plan: volume is reduced and the remaining breast is reshaped, with nipple position adjusted as needed. A breast lift alone focuses on shape and position without substantial volume removal.

Light routines may be possible in roughly one to two weeks, but work and activity clearance depends on the operation and healing. The contour continues to settle for several months.

Return to activity is gradual and requires individual clearance from your surgeon:

1. First week: allow time for rest, wound care and help with daily tasks.
2. Following weeks: return to work depends on your healing and whether your job involves lifting or physical effort.
3. Six weeks and beyond: discuss exercise and scar care at review; swelling and the final appearance may take several months to settle.
